Paulinho's late header put Brazil into the Confederations Cup final at the expense of neighbours Uruguay.
The Corinthians midfielder steered in Neymar's corner at the far post to spark jubliant celebrations inside the Estadio Mineirao in Belo Horizonte.
Brazil led at half-time through Fred's scuffed scissor-kick before Edinson Cavani steered in an equaliser after some slack home defending.
Uruguay's Diego Forlan saw his low penalty saved before Fred's opener.
Brazil now face either Spain or Italy, who meet in the other semi-final on Thursday, in Sunday's final in Rio de Janeiro.
